This could be either the request or the reply
getting lost. The way to find out is to check
the replies using a completely separate receiver
or any other way to monitor a physical midi stream.

This could well be due to entirely different causes as the
first problem.
 
I'd first try to get the version with the usleep() working,
and then attack the other problem.

Could you try to remove *all* interaction with gtk in
midi_callback(). Just print the received events with
printf() and run your app from a terminal.
