Then PureData might be of help. Try to get a hold of the Theremin patch
(http://cmpercussion.blogspot.com/2008/08/theremin-to-midi-control-program-in-pd.html)
and with a little tinkering it should do what you describe.
Maybe it can be done with Rakarrack too, not sure, I've never really
used Rakarrack's audio to MIDI functionality.
