That was kinda usefull. I realized that I forgot to run update-modules
after editing /etc/modutils/alsa. After running that the files I posted
almost does what I need.
But this (long) line did the trick:
options snd-usb-audio index=1,3,4,5 vid=0x0582,0x0a4d,0x0a4d,0x0a92
pid=0x0018,0x008e,0x008e,0x1000\ id=ua-1a,evolution1,evolution2,midimate
