I got the very same problem after upgrading to mandriva 2007.

installed the new version of qjackctl from sources (0.2.21) and it
improved the midi tab.
more likely the original poster doesn't have the snd-seq kernel module
loaded. lsmod is the diagnostic tool here. for some reason, lots of
distros fail to load this module by default.
nope. he said he explicitly loaded it. trouble is mandriva 2007's
qjackctl package which seems to have been built without alsa support.
