Freeswitch[0] fits this profile. It can function as a SIP[1] voip
phone and it supports pulseaudio, which you can hook up as a jack
client. Everything is command based.

Using the jabber (xmpp) jingle extension could also be an option. There
are quite some clients to choose from on every platform.
Main problem is that jingle isn't supported by many clients yet, but
I guess it will be soonish.
