Since issue doesn't occur 100% of the time, maybe
it's something
specific to projects that have the problem - like a particular plugin or
softsynth patch?

I had this happen some time ago with ardour as well. I mentioned it to
las in the IRC, and he said it would take some special code to track
down the issue.
That said, it hasn't happened to me in many months.
It has yet to happen using Mixbus.
